How to conduct FMEA of flame detector?

How to conduct FMEA of flame detector?

=Method can help manufacturers establish the reliability of product designs by carrying out a Failure Modes and Effects Analysis (FMEA).

The results can be used to forecast the reliability of the design, and from this information the design may be improved or the data used to determine the safety and availability performance when the device is incorporated in to a larger system.

=Method was asked to conduct such a review on a flame detector.

Working component by component, we identified all the potential failures that could occur, analysed what effect each failure had on the performance of the product and then, based on reliability data for each component, established rates of safe and dangerous failures.

Our project resulted in us establishing failure data and failure rates for the product, we also made a number of recommendations for changes to documentation and highlighted some conditions of use.

Functional Safety: More Than a PFD Calculation

Functional Safety design puts a lot of emphasis on the reliability of SIF’s and SIS’s via PFD average calculation. In this presentation Simon Sheehan, MD of Method Functional Safety will highlight some other steps of the lifecycle which are often overlooked, or poorly executed, sometimes negating the PFDavg/reliability of the SIF completely.
